What This Test Is For

Penile duplex ultrasound is a non-invasive diagnostic test used to evaluate blood flow in the penis. It helps identify vascular causes of erectile dysfunction (ED), assess priapism, and monitor recovery after penile surgery or trauma.

Who Should Consider It

This test is recommended for men with erectile dysfunction of unknown cause, suspected vascular disease, penile trauma, Peyronie’s disease, or those preparing for penile prosthesis surgery. It is also used to evaluate prolonged erections (priapism).

What to Expect During the Procedure

You will lie in a comfortable position while ultrasound images are taken at baseline. In many cases, a small dose of medication is injected into the penis to produce an erection, allowing measurement of peak arterial flow and venous function. The procedure typically takes 20–30 minutes.

How to Prepare

No fasting is required. Avoid erectile medications (Viagra, Cialis) 24 hours before the test. If a pharmacologic erection is planned, do not use nitrates and arrange transportation, as the erection may take time to subside before leaving.

Results may show normal blood flow, arterial insufficiency (reduced inflow), or venous leak (poor blood retention). These findings guide personalized treatment options for ED.
